Print and fill sheet of paper with values

Hi there guys 

I really need help with this work for my school the thing i need to do is just fill with values a sheet of paper, the user will fill the textboxes in the ASP.nET applicacion and then i will call the aplicaction to pirnt the format, But the thing is that the format is already done, I mean its an official paper so i cant print it again with the values.

Now imagine that this is the official format, the size is LETTER: 


NOW the user will fill the values in the application and HE will print it and it has to appear like this:




I can do this just recreating the format but i cant becuase the sheet of PAPER IS OFFICIAL SO I JUST WANT TO FILL THE SHEET WITH VALUES FROM THE PRINTING OF THE APPLICATION.
