Requirement: To create Web Forms

Hello, I have a requirement for an existing website, and while I need to focus on steps 1, 2, and 3, below, for now, I included the remaining steps, 4 and 5, so that however, the first three steps are developed, they will fit into the the entire requirement, continued in steps 4 and 5, 1) I need to build a form on a new web page that is an inquiry. 2) The form input fields will need drop downs for the users to choose their input. 3) The values that the users can choose from within the drop downs need to be created ahead of time in secure database tables, first, and interfaced with the inquiry. 4) When the user submits the form data, their information should be stored in secure two tables, one a customer master table, the other a table that will store the transactional record of their input. 5) While the client is requesting a cloud- based service, I don't think it matters if the databases are stored in the cloud or n ...
