Creating Survey For Consumer Confidence Index

I need to create a survey for finding CCI, so can someone tell me how to create a survey. Also survey responses should be stored in database in such a way that I can calculate CCI on the responses. At the end the calculated CCI values should be displayed in form of graphs,piecharts.
