Working with multiple textboxes

hi,
I have one text box and one button when i click on button textbox add in page on every single click i want a code how to remove a text box just like add button i want remove button that remove text box on every single click.i want syntax in jquery.
Also, I have one text box name item . i have a button when i click to button multiple textbox show every single click text box show like first is ITEM #1 when i click to button another text box show like ITEM #2 i have to insert that in database. can any one provide me a syntax.
