Save the resume in local folder and calendar display problem

i have carreer form in my page.in that to enter date of birth i have displayed a calendar.

but the problem is if i used single text box its working fine.but iam using 3 boxes for date,month,year.how can i assign it 3 text boxes.

i have another issue in that page user post his resume also..for that i placed file upload control.

plz help me in saving the form with word document.my requirement is i want to display the word document also for recruiters..plz help me in doing this.. how to save the word document.which is the right way either saving the form in localfolder or saving the entire documnet in databse..and how to dispaly to the recruiters
